* Physically enter the City Clerks office
* Present a picture identification of yourself
* Inform the clerk if you want a wallet or full size certificate
* To request a birth record by mail
* State full name and date of birth for certificate and enclose identification of yourself
* State wallet or full size
* Enclose payment along with written request on the download form
* Enclose a self-addressed envelope
* If request is for a record other than your own, follow the above and also state your relationship to that person.
